An Air Force observation plane discovered flame in April, jeopardizing the lives of 27 aviators on board the plane - all on the grounds that a holding nut joining oxygen tubing was not fixed appropriately, mischance agents have decided. The report faulted a private barrier contracting organization for the mishap.
"Disappointment by L-3 Communications terminal support faculty to fix a holding nut uniting a metal oxygen tube to an intersection fitting over the cookroom appropriately brought about an oxygen spill. This release made a very combustible oxygen-rich environment that touched off," U.S. Flying corps agents wrote in report distributed August 3.
Agents decided the resulting flame brought on $62.4 millon in harm to the RC-135V, which electronically snoops on foes and transfers assembled insight to officers.
